Discharge Ready Date

Discharge Ready Date is the date that a PATIENT no longer met the 'Criteria to Reside' in a Hospital Bed.

Description

Discharge Ready Date is an ACTIVITY DATE TIME.

Discharge Ready Date is the date that a PATIENT no longer met the 'Criteria to Reside' in a Hospital Bed.  Note that Discharge Ready Date  is not the same as the Estimated Discharge Date (Hospital Provider Spell)  and is recorded separately.

The status of a PATIENT within a Hospital Provider Spell  who is awaiting discharge may change from no longer meeting the 'Criteria to Reside' in a Hospital Bed, to once again meeting the 'Criteria to Reside' in a Hospital Bed, if the clinical condition of the PATIENT  deteriorates while awaiting discharge for any reason.

The 'Criteria to Reside' are set out in Annex D of the 'Hospital discharge and community support guidance' document part of the gov.uk website at: Criteria to Reside.

Usage in the Commissioning Data Sets  version 6-2 and Commissioning Data Sets  version 6-3:

For Finished Care Professional Admitted Care Episodes, the Discharge Ready Date should record the start date of the final period that the PATIENT no longer met the 'Criteria to Reside' in a Hospital Bed for that episode of care.

For Unfinished Care Professional Admitted Care Episodes, the Discharge Ready Date should record the start date of the current period that the PATIENT no longer met the 'Criteria to Reside', if applicable for the current episode of care.
