REFERRAL CLOSURE REASON FOR COMMUNITY CARE

The REFERRAL CLOSURE REASON FOR COMMUNITY CARE indicates the reason that a REFERRAL REQUEST has been closed.  A REFERRAL REQUEST  can be closed on rejection of the REFERRAL REQUEST by the Community Health Service, or as a result of a PATIENT being discharged from the Community Health Service.
